About the role
We are currently seeking a skilled and motivated Primary Care Physical Therapy Assistant to join our dynamic team. As a valuable member of our healthcare staff, you will play a crucial role in assisting our licensed physical therapists in delivering exceptional care to our homebound patients.
Responsibilities
- Perform visits in homes, ALF's, and ILF's under the supervision of licensed physical therapists.
- Assist in the implementation of treatment plans, conducting exercises with patients, and educating patients on proper techniques for rehabilitation exercises.
- Document patient progress and communicate regularly with physical therapists and other healthcare professionals to coordinate patient care and adjust treatment plans as needed.
- Develop and instruct patients on individualized treatment plans, assist with therapeutic exercises and activities, and monitor patients during therapy sessions, providing guidance and support as needed.
- Maintain a clean and organized therapy environment, ensuring that equipment is properly sanitized and maintained.
- Aid in the implementation of discharge plans, providing patients with instructions for continued care and recommending appropriate follow-up services.
Requirements
- Associate degree from an accredited Physical Therapy Assistant program.
- Current licensure or eligibility for licensure as a Physical Therapy Assistant in the state of practice.
- CPR certification.
- Strong interpersonal and communication skills.
- Ability to work effectively as part of a multidisciplinary team.
- Excellent organizational and time management skills.
- Compassion and empathy for patients undergoing rehabilitation.
- Physical stamina and dexterity necessary to assist patients with mobility and perform therapeutic activities.
- Pelvic Health Experience and/or interest (preferred).
Qualifications

This is a salary-based position, with the opportunity to receive a monthly bonus.
Schedule
This is a full-time, salary-based position. There are two schedules available:
- Monday-Friday (8:00AM-5:00PM)
- Monday-Sunday (7:00AM-7:00PM), 7 days on, 7 days off
