Physical Therapist Weekend

Schedule

• Sat/Tues/Thurs, 8:30am–5pm
• 8 patient visits (PIQs) per day
• On-call weekend per rotation
• 1 holiday per year

Territory: Kettering and surrounding area.

How You'll Make a Difference

Care and compassion are at the heart of what you'll do as a Physical Therapist (PT). You'll be part of an interdisciplinary team that focuses on providing compassionate quality care and producing positive outcomes for your patient population. You'll provide home-based therapy care for patients as directed by the attending physician. Interacting with patients' families while caring for your patients and experiencing the rewarding privilege to be part of every step of their recovery journey. Responsibilities

Plan of Care

• Provide skilled physical therapy as ordered by the attending physician
• Observe, record, and report patient reactions, changes, and progress to attending physician and agency staff
• Perform OASIS assessments to develop an individualized plan of care and adjust as needs change

Patient Outcomes

• Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means such as exercise, massage, heat, water, etc.

Patient Risk

• Help decrease re-hospitalizations by front-loading visits for high-risk patients
• Teach patients preventive measures and decision-making skills

Family Educator/Advocate

• Counsel patients and families on disease/injury management through all phases of physical therapy
• Instruct on proper use of equipment (wheelchairs, braces, walkers, crutches, canes, prosthetic/orthotic devices)

Supervision

• Oversee and supervise therapy services provided by PTAs and nurse assistants

Communication/Collaboration

• Communicate supervisory visit results with PTAs and nurse aides

Policies

• Complete all clinical documentation following agency protocol and Medicare/Federal guidelines

Rules and Regulations

• Understand and follow agency policies, procedures, rules, and regulations
• Communicate changes in schedule/availability to schedulers or supervisors

Operations

• Attend in-service training and mandatory agency meetings

Skills

• Compassionate communicator with a positive attitude
• Patience when working with patients, families, physicians, and coworkers
• Attention to detail, observant, and able to follow directions

Requirements

• Current Physical Therapist license in the state of employment
• Minimum one-year experience as a PT in an acute care setting
• Home care experience preferred
• Must work 1 dedicated weekend day per week
• Valid driver’s license and auto insurance in your name as a driver
• Capable of all physical demands of the role
