About the role

Join a traveling Physical Therapist team serving Wallingford, CT and surrounding towns where you will help patients recover from injuries and manage chronic conditions while working in a home health setting.

Responsibilities

  • Help patients recover from injuries and manage chronic conditions in their homes.
  • Document visits electronically.
  • Coordinate care across interdisciplinary teams.
  • Maintain continuity of care across diverse patient populations.
  • Collaborate with interdisciplinary teams to support patient recovery and independence.

Requirements

  • Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Connecticut Physical Therapy license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Experience in home health delivery preferred.
  • Candidates must be able to manage caseloads in patients' homes and document visits electronically.
  • Valid driver's license and reliable transportation required for travel between patient homes.
  • Mileage between visits will be reimbursed.
  • Minimum education requirement: Bachelor's Degree. Degree in Physical Therapy required.
  • Ability to work independently and coordinate care across interdisciplinary teams.
  • Previous experience with home health documentation and care planning preferred.

Schedule

Days 0800-1600. Weekly Hours Guarantee: 32 hours per week. On Call/Weekend Requirement: No.
