Responsibilities

• Help patients recover from injuries and manage chronic conditions.
• Document in electronic medical records and follow Medicare skilled documentation standards.
• Manage restorative programs and use common mobility and therapeutic modalities.

Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active State Physical Therapy license (ID or compact license valid for assignment).
• BLS certification from the American Heart Association or American Red Cross.
• Experience in skilled nursing facility (SNF) settings preferred.
• Prior SNF caseload management and restorative program experience are preferred.
• Reliable professional references demonstrating recent clinical experience.

Qualifications

• Minimum years of experience may be requested by the client; candidate should be prepared to provide detailed work history.

Skills

• Experience with documentation in electronic medical records and Medicare skilled documentation standards.
