About the role
The reputable Skilled Nursing Facility is seeking a highly motivated and energetic Physical Therapist to join their team in Eureka, CA.
Responsibilities
- Provide therapeutic interventions to promote functional independence and improve quality of life for patients.
- Develop and implement individualized treatment plans based on patient needs and goals.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Evaluate patient progress and adjust treatment plans as necessary.
- Document patient care activities accurately and timely.
Requirements
- Physical Therapist, Skilled
- Experience: 1 year (New Grad accepted)
- Preferred: Active CA License
- Travel Experience Preferred
Qualifications
- Current active license to practice Physical Therapy in California
- Passing score on the National Physical Therapy Examination (NPTE)
- Completion of an accredited Physical Therapy program
Skills
- Strong clinical skills
- Effective communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in computer software for documentation and patient management
