About the Role
Seeking a licensed Physical Therapist to provide age-appropriate hospice services, including assessments, direct treatments, patient education, adaptive equipment evaluation, and documentation in accordance with hospice standards.
Responsibilities
- Perform physical therapy evaluations based on physician orders and Plan of Care to manage symptoms and support daily functioning
- Document visits, patient responses, and changes in condition; communicate updates to physicians and administrators
- Participate in the Interdisciplinary Group, contributing to the patient's Plan of Care
- Educate patients, families, and team members on functional limitations and progress towards goals
- Prepare discharge summaries and maintain accurate clinical records
- Instruct patients, families, and team members on physical therapy techniques when appropriate
- Adapt to various home environments, ensuring quality care regardless of client demographics
Requirements
- Current Physical Therapy licensure in the state
- CPR certification or ability to obtain within 90 days
- Valid driver’s license, vehicle insurance, access to transportation, or public transit

Pay
$85,120 to $127,680 annually based on full-time employment.
