About the role

Join a dedicated team as a Physical Therapist, responsible for planning, organizing, and implementing Physical Therapy services for participants in a comprehensive care program.

Responsibilities

• Conducting assessments and developing individualized treatment plans in collaboration with an interdisciplinary team.
• Administering physical therapy treatments to help participants achieve optimal independence.
• Assessing needs for adaptive equipment, home safety modifications, and specialized footwear or prostheses.
• Educating participants and their families on procedures, home programs, and safety practices.
• Training staff on body mechanics, equipment use, and transfer techniques to enhance safety and function.
• Documenting evaluations, treatments, and progress accurately and timely, and revising plans as needed.
• Participating in team meetings, quality improvement initiatives, and community or organizational events.
• Ensuring compliance with policies, safety standards, confidentiality, and continuing education requirements.

Qualifications

• Advanced degree in Physical Therapy from an accredited institution.
• Current state licensure to practice Physical Therapy.
• Valid driver’s license and auto insurance.
• Preferred experience working with elderly or frail populations and interdisciplinary teams.
• Ability to collaborate effectively, adapt to team needs, and provide high-quality patient-centered care.
