About the role

Full-Time Home Health Physical Therapist (PT) providing patient assessments, developing treatment plans, and delivering interventions to improve patient mobility, pain management, and overall well-being.

Responsibilities

• Evaluating patients' functional status and therapy needs.
• Assisting in developing and implementing personalized care plans.
• Providing treatment within legal scope and coordinating with healthcare team.
• Monitoring and reporting patient progress and response to treatments.
• Educating patients, families, and healthcare team members about care plans and progress.
• Completing evaluations, progress notes, and reports per policy and guidelines.
• Adapting to various home environments and ensuring respectful, equitable care.

Requirements

• Current Physical Therapy license in California.
• Valid driver’s license, vehicle insurance, or access to transportation.
• Preferred: CPR certification or ability to obtain within 90 days.

Pay

Salary ranges from $97,888 to $146,832 annually based on experience.
