About the Role

The Physical Therapist in Home Health evaluates patients' functional status, mobility, endurance, and pain to develop personalized treatment plans. They provide therapeutic interventions to improve patient well-being, restore function, and relieve pain, working within state laws and in coordination with the healthcare team. Additionally, they educate patients, families, and team members on care plans, monitor progress, and document visits and outcomes.

Responsibilities

• Assess patient needs and assist in developing therapy plans with physicians
• Deliver services within legal and professional scope, observing patient responses
• Treat patients to enhance mobility, reduce pain, and maintain optimal function
• Educate patients, families, and healthcare team members about care plans and progress
• Complete evaluations, develop care plans, and document visits and outcomes
• Adapt to diverse home environments, ensuring equitable care regardless of background

Requirements

• Current Physical Therapy license in the practice state
• Valid driver’s license, vehicle insurance, or access to reliable transportation

Preferred Qualifications

• CPR certification or ability to obtain within 90 days
