Physical Therapist (Sign On Bonus)

About the role

Licensed Physical Therapist providing personalized, in-home and senior living care to older adults, utilizing a flexible schedule and virtual care options.

Responsibilities

• Conduct evaluations and develop individualized treatment plans.
• Deliver evidence-based therapy to support patient mobility, independence, and wellness.
• Document care in EMR systems and collaborate with an interdisciplinary team.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Support virtual care when appropriate to enhance access and continuity of care.

Qualifications & Skills

• Valid PT license and graduation from an accredited program.
• Proficiency in EMR and basic computer skills.
• Current CPR certification.
• Physical ability to perform patient transfers, repositioning, and extended walking; ability to exert medium effort and drive to patient locations.
