About the role

Provide in-home, senior living, and virtual care to older adults, primarily in Manalapan, NJ.

Responsibilities

• Conduct evaluations and develop personalized treatment plans.
• Deliver evidence-based therapy to enhance mobility, independence, and wellness.
• Document care accurately and collaborate with interdisciplinary teams.
• Educate patients and caregivers.
• Incorporate telehealth when appropriate.

Qualifications & Skills

• Valid Physical Therapist license or eligibility.
• Degree from an accredited Physical Therapy program.
• Basic EMR and computer proficiency.
• Current CPR certification.
• Ability to meet physical demands including lifting and patient assistance.
