About the Role

As a Full-Time Home Health Physical Therapist (PT), you will assess patient needs, develop treatment plans, and implement interventions to improve patient mobility, reduce pain, and maintain functional abilities in home settings.

Responsibilities

• Evaluate patients' functional status and assist in creating therapy plans of care.
• Provide treatment within scope of practice, coordinate with the healthcare team, and monitor patient progress.
• Report patient responses and changes to supervising healthcare professionals.
• Educate patients, families, and team members on care plans and progress.
• Complete evaluations, develop plans of care, and document visits and outcomes.
• Adapt to various home environments, ensuring quality care regardless of patient demographics.

Requirements

• Current Physical Therapy license in California.
• Valid driver’s license, vehicle insurance, or access to transportation.
• Preferred: CPR certification or ability to obtain within 90 days.

Pay

Compensation ranges from $97,888 to $146,832 annually.
