About the role

Join a clinician-owned organization as a Home Health Physical Therapist, providing personalized rehabilitation care in patients' homes within a high-travel role.

Responsibilities

• Assess patients’ functional abilities and home environments
• Develop and implement individualized treatment plans
• Guide interventions to improve mobility, strength, balance, and safety
• Educate and empower patients and caregivers
• Collaborate with an interdisciplinary care team to achieve recovery goals

Qualifications

• Master’s or doctoral degree in Physical Therapy from an accredited program
• Current state license and at least 1 year of experience
• Reliable transportation and willingness to travel daily within the service area

Preferred: Home health experience, OASIS certification, Medicare documentation experience
