Physical Therapist (Sign-On Bonus!)

Role & Responsibilities

• Provide one-on-one physical therapy to older adults in private residences and senior living communities.
• Conduct evaluations, develop personalized care plans, and deliver evidence-based treatments to enhance mobility, independence, and quality of life.
• Document patient progress accurately within the EMR system and collaborate with an interdisciplinary team of healthcare professionals.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Incorporate telehealth and virtual care to increase flexibility and patient access.

Qualifications & Skills

• Valid Physical Therapy license or eligibility in the practice state.
• Degree from an accredited PT program.
• Proficiency with computers and EMR systems.
• Current CPR certification.

Physical & Additional Requirements

• Ability to perform moderate physical effort, assist with patient transfers, and meet in-home care demands, including driving and PPE use.
