Physical Therapist (Sign-On Bonus!)

About the Role

Provide personalized, one-on-one physical therapy to older adults in private homes or senior living settings. Utilize telehealth and virtual care options to increase flexibility and patient access.

Responsibilities

• Perform evaluations and develop individualized treatment plans.
• Deliver evidence-based therapies to enhance mobility, independence, and quality of life.
• Document care effectively using electronic medical records.
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff to promote safety and aging-in-place strategies.

Requirements

• Valid PT license or eligibility in the practice state.
• Degree from an accredited physical therapy program.
• Proficiency in basic computer use and EMR systems.
• Current CPR certification.
• Ability to meet physical demands such as lifting, repositioning patients, standing for long periods, and driving to patient locations.
