About the role

Provide personalized, in-home and senior living care, including virtual sessions, to older adults in private residences and community facilities.

Responsibilities

• Conduct evaluations and develop individualized treatment plans
• Deliver evidence-based therapy to enhance mobility, independence, and quality of life
• Document patient care accurately within electronic medical records
• Collaborate with an interdisciplinary team of healthcare professionals
• Educate patients, caregivers, and staff on safety and aging-in-place strategies
• Provide telehealth and virtual care when appropriate

Requirements

• Valid PT license or eligibility in the practice state
• Degree from an accredited PT program
• Proficiency in computer and EMR systems
• Current CPR certification

Physical requirements

• Ability to perform moderate physical effort
• Assist with patient transfers and reposition patients
• Drive to various care locations
