Physical Therapist – Norwalk, CT Seeking passionate licensed Physical Therapists to deliver one-on-one, patient-centered care to older adults in private residences and Senior Living Communities. This role emphasizes autonomy, flexibility and meaningful patient outcomes, allowing you to create your own treatment schedule and foster strong relationships. Clinicians may incorporate telehealth and virtual care to enhance access and continuity of care. Responsibilities Include Providing individualized physical therapy to seniors in their homes or communitiesPerforming evaluations, developing care plans, and delivering evidence-based treatmentsSupporting wellness, mobility, and independence through therapy sessionsDocumenting effectively within the EMR systemCollaborating with an interdisciplinary team of PTs, OTs, SLPs, and Fitness SpecialistsEducating patients, caregivers, and staff to promote safety and aging in placeOffering telehealth/virtual care when appropriate Qualifications Include Valid Physical Therapy license or eligibility in the stateDegree from an accredited PT programProficiency with computers and EMR systemsCurrent CPR certification Physical requirements: lifting, repositioning, assisting with transfers, extended walking, and driving to patient locations, with PPE use when necessary.
