Physical Therapist (PT, Therapy, Rehab) – Homecare

About the role

The Physical Therapist at Hartford HealthCare at Home is responsible for administering medically prescribed Physical Therapy treatments to patients. They independently evaluate patient’s level of function, determine treatment strategies, and deliver or delegate Physical Therapy services to maximize the patient's functional independence.

Responsibilities

• Evaluates patient’s level of function, strength, movement, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function
• Performs therapeutic techniques that are appropriate to patient’s needs and medical status
• Completes all documentation including but not limited to: evaluation, care plan and goals, patient status and progress, visits, patient discharge summary, and patient care transition plan and summary
• Participates in patient transition planning and/or patient discharge planning including recommendations to community resources to meet patient needs throughout the continuum of care
• Helps to coordinate patient care with other care providers such as Rehab Therapists, Nurses, Social Workers, and other community resources as related to care transition planning

Requirements

• Bachelor's degree in Physical Therapy
• Current State of CT Physical Therapy license
• Basic Life Support (BLS)
• Valid driver’s license and reliable transportation required for Home Care positions

Qualifications

• Preferred Master’s Degree in Physical Therapy or Doctor of Physical Therapy
• Previous experience as a physical therapist

Schedule

Mon thru Friday. Coverage for Torrington area. Can consider 32-40 hours. Requires coverage 1 weekend day a month, 1 major and 1 minor holiday.
