Physical Therapist - Homecare

About the role

Park Nicollet is seeking a Physical Therapist to join our Home Care/Hospice team in St. Louis Park. This position offers the opportunity to be a Partner for Good, making a positive impact on the care and experiences of patients and their families.

Responsibilities

• Access and evaluate patient care needs
• Provide skilled therapy care and teaching to patients and families in their home
• Improve and prepare patients and families for self-directed and/or assisted care in their community setting
• Coordinate medical, social, and financial resources to develop a comprehensive plan of care
• Support community care processes including admission, discharge, referrals to other disciplines, billing, and care delivery

Requirements

• Bachelor's, Master's, or Doctorate degree in Physical Therapy
• Current license with Minnesota Board of Physical Therapy
• Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained
• On-line BLS certification is not acceptable
• Work experience as a Physical Therapist for at least 3 years preferred
• Experience in homecare desirable with knowledge of OASIS data set

Pay

$39.73 - $59.59 per hour

Schedule

Monday – Friday 8a-4:30p. Occasional weekends.
