Athletico believes in supporting extraordinary people who transform lives through life-changing care. As a Physical Therapist, you'll play a pivotal role in evaluating, designing, and implementing a patient's plan of care to enhance their function and independence.

Responsibilities

  • Assess and screen patients' daily living/work-related skills and develop therapeutic retraining programs with measurable goals.
  • Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings.
  • Confer with the patient's physician and other healthcare team members to assist in developing/revising the Plan of Care.
  • Provide therapeutic treatment and instruction to patients in accordance with physician orders.
  • Review and expand the treatment programs in line with the functional goals of each patient and within the guidelines of the rehabilitation program.
  • Report patient status and progress to the physician, rehabilitation staff, patient, and family.
  • Participate in care coordination and discharge planning activities and serve as a resource to other healthcare personnel.
  • Recommend and/or design special adaptive equipment for clients to improve their residence or working environments or improve their participation in the rehabilitation program and/or community.
  • Train family members and caregivers to carry out the treatment including dressing, feeding, grooming, and hygiene skills, participate in team and family educational meetings.
  • Accurately and thoroughly document patients' care observations, interventions, and evaluations on the day services are provided.
  • Ensure that interim (verbal) orders received from the physician are accurately documented and implemented.
  • Submit evaluation, treatment plans, and discharge summaries within twenty-four (24) hours.

Qualifications

  • Education: Degree from an accredited Physical Therapy Program.
  • Work Experience: A minimum of 5 years of physical therapy experience preferred, with home health experience preferred.
  • Certification/Licensure: Current Physical Therapist licensure in good standing.
  • Knowledge and Technical Skills: Current CPR certification, proof of a negative TB test recommended.
  • Exceptional organizational and communication skills.
  • Current and valid driver’s license and automobile insurance.
