Physical Therapist (PT), PRN

About the role

Chandler Creek Post Acute is a 133-bed skilled nursing facility in Greer, South Carolina. We are seeking a motivated Physical Therapist (PT) to join our team on a PRN basis.

Responsibilities

• Evaluate and treat residents to improve mobility, function, and overall quality of life
• Develop and deliver therapeutic interventions to enhance strength, balance, and mobility
• Maintain accurate and timely documentation of patient progress
• Collaborate with interdisciplinary teams to support patient outcomes
• Provide education to residents and families on therapy techniques and recovery goals

Qualifications

• Bachelor’s degree required; advanced degree preferred
• Active, unencumbered Physical Therapist license in South Carolina
• Experience in a long-term care or rehab setting preferred
• Familiarity with PointClickCare (PCC) and Casamba preferred
• Strong communication and clinical skills
• Ability to work independently and as part of a team
