About the role

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our rehabilitation team on a PRN basis.

Key Responsibilities

  • Evaluate residents and develop individualized treatment plans
  • Provide physical therapy services in accordance with physician orders and established goals
  • Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and timely
  • Maintain resident progress and modify treatment plans as appropriate
  • Communicate with physicians, nursing staff, residents, families, and other healthcare professionals regarding treatment plans and outcomes
  • Participate in care conferences, rehabilitation meetings, and discharge planning
  • Educate residents, families, and caregivers on therapy goals and home exercise programs
  • Conduct in-service training and education for facility staff as needed
  • Supervise Physical Therapist Assistants (PTAs), aides, and students in accordance with state regulations
  • Ensure compliance with state practice acts, facility policies, and regulatory requirements

Qualifications

  • Education & Experience: Bachelor's degree in Physical Therapy required. Master's or Doctorate degree in Physical Therapy preferred. Experience in skilled nursing, rehabilitation, or long-term care preferred. Experience with Casamba and PointClickCare (PCC) preferred.
  • Licenses & Certifications: Current South Carolina Physical Therapist license in good standing. Must maintain all required continuing education requirements. Must remain in good standing with the State Board of Physical Therapy.
