Physical Therapist (PT), PRN

About the role

Piedmont Post Acute is an 88-bed skilled nursing facility in Piedmont, South Carolina. We are seeking a compassionate and skilled Physical Therapist (PT) to join our rehab team on a PRN basis.

Responsibilities

• Evaluate and treat residents to improve mobility, strength, and overall function
• Develop and implement individualized treatment plans
• Provide therapeutic interventions to improve mobility, strength, and balance
• Monitor progress and adjust treatment plans as needed
• Maintain accurate and timely documentation
• Collaborate with interdisciplinary team members
• Inform residents and families about treatment plans and goals

Requirements

• Active, unencumbered Physical Therapist (PT) license in South Carolina
• Education in Physical Therapy (Bachelor’s, Master’s, or Doctorate)
• Experience in long-term care or rehab setting preferred
• Familiarity with PointClickCare (PCC) and Casamba preferred
• Strong communication and clinical skills
