Responsibilities Evaluate patients and assess physical conditions, mobility, and functional limitationsDevelop and implement individualized treatment plansProvide therapeutic exercises, stretching, and hands-on treatmentMonitor patient progress and modify treatment plans as neededDocument evaluations, treatments, and patient progress accuratelyEducate patients and families on therapy goals, home exercises, and recoveryCollaborate with nursing and other members of the interdisciplinary care team Qualifications Doctor of Physical Therapy (DPT) from an accredited programCurrent New Jersey Physical Therapist licenseMinimum 2 years of experience preferredSkilled nursing or nursing home experience preferredStrong clinical assessment, communication, and interpersonal skillsFamiliarity with SigmaCare and Net Health preferred Apply Today!
