Responsibilities
- Conduct comprehensive evaluations and develop individualized treatment plans based on resident goals and clinical needs
- Provide skilled, evidence-based physical therapy treatments in accordance with the California State Practice Act
- Supervise Physical Therapist Assistants (PTAs) and support Restorative Nursing Aides to ensure continuity of care
- Maintain timely and accurate documentation, including daily treatment notes, progress reports, and recertifications
- Collaborate closely with nursing staff and the interdisciplinary care team; participate in care planning meetings and utilization reviews as needed
- Educate residents and families on mobility safety, home exercise programs, and discharge planning
- Recommend and coordinate durable medical equipment (DME) such as walkers, wheelchairs, and braces to support successful recovery
Requirements
- Current and unrestricted California Physical Therapist (PT) license required
- Strong clinical judgment and documentation skills
- Excellent communication and interpersonal abilities
- Ability to motivate and engage residents throughout the rehabilitation process
- Positive, team-oriented attitude with a passion for helping others regain independence
