About the role

As a Physical Therapist in the Program of All-Inclusive Care for the Elderly (PACE), you will build long-term therapeutic relationships with elderly participants, helping them maintain independence and remain in their communities. Unlike acute or post-acute settings, PACE allows you to follow patients over time, see the cumulative impact of your interventions, and truly get to know the people you serve.

Responsibilities

  • Provide comprehensive physical therapy evaluations and treatments for PACE participants in a community-based setting.
  • Focus on fall prevention, mobility optimization, pain management, and functional maintenance.
  • Collaborate with an interdisciplinary team including physicians, nurses, social workers, and occupational therapists to develop holistic care plans.

The pace of work is steady and sustainable, with an emphasis on quality over volume.

Requirements

  • DPT or Master's degree from an accredited PT program.
  • Current or eligible for Pennsylvania State PT license.
  • Interest in geriatric care and community-based health.
  • Strong assessment and communication skills.

New graduates are welcome.

Pay

Competitive pay with rate ranges based on position, years of applicable experience, and employment type. Rates include levels for per diems with no benefits and Independent Contractors.
