About the Role

Responsible for coordinating and delivering physical therapy services, including evaluating patients, developing individualized treatment plans, and providing direct therapy in various settings.

Responsibilities

• Assess patients through data collection and clinical observation.
• Design and implement personalized treatment programs, educating patients and caregivers.
• Use functional activities to promote health and adapt interventions based on ongoing evaluations.
• Document evaluations, treatment plans, progress notes, and communicate with the healthcare team.
• Collaborate with interdisciplinary team members and participate in care conferences.
• Supervise and oversee physical therapy assistants, techs, and students to ensure safe, effective patient care.
• Maintain knowledge of equipment and current therapy practices.

Requirements

• Active physical therapy license in the practice state.
• Preferred: licensed physical therapist.
• Must meet visual and communication requirements; accommodations available for disabilities.
