Responsibilities

- Supervise support personnel

- Maintain documentation
- Collaborate with the rehabilitation team
- Perform home and community mobility assessments
- Adapt therapy programs accordingly
Qualifications
- Valid state license
- Master's degree in Physical Therapy or equivalent
- Reliable transportation
