About the role

The Physical Therapist provides outpatient rehabilitation services in-home for older adults, assessing and developing personalized therapy programs to promote independence, mobility, and health.

Responsibilities

• Evaluate patients
• Create treatment plans
• Instruct patients and caregivers on therapeutic techniques
• Supervise support staff
• Ensure documentation compliance
• Made interdisciplinary referrals
• Maintain equipment
• Implement various therapeutic modalities
• Fabricate adaptive devices
• Perform mobility assessments
• Collaborate with the healthcare team to optimize patient outcomes
• Adapt plans based on individual needs

Qualifications

• A valid state licensure in Physical Therapy, with a Master's degree in Physical Therapy or equivalent
• Candidates with relevant experience or foreign credentials evaluated as equivalent are considered
• Must have reliable transportation and a valid driver’s license
