Physical Therapist (PT) - Outpatient Mobile

Role Overview

Seeking a Physical Therapist to provide outpatient rehabilitation services primarily in-home for the older adult population, including private residences and independent living facilities.

Responsibilities

• Assessing patient needs
• Developing and implementing personalized therapy programs to restore and enhance physical function
• Educating patients and caregivers to promote independence and correct impairments
• Supervising support personnel, ensuring documentation compliance, and participating in care team meetings
• Making referrals to other disciplines and adapting treatment plans based on patient progress
• Designing and fitting adaptive devices
• Performing mobility assessments
• Promoting health and wellness
• Maintaining equipment, ensuring safety, and delivering patient-centered care in various settings

Qualifications

• Valid PT license in the practice state or eligibility
• Master's degree in Physical Therapy or equivalent, or Bachelor's plus five years of experience
• Reliable transportation and a valid driver’s license
