Physical Therapist (PT) - Outpatient

Responsibilities

• Provide patient-centered in-home outpatient therapy, assessing needs and developing individualized treatment plans.
• Perform evaluations, instruct patients and caregivers, and make referrals to other disciplines as needed.
• Implement therapeutic techniques including therapeutic exercises, manual therapy, modalities, and adaptive device training.
• Supervise support personnel, ensure documentation accuracy, and adhere to treatment schedules.
• Maintain a safe environment, order supplies, and perform routine equipment maintenance.
• Participate in team meetings, case management, and community mobility assessments.

Qualifications & Skills

• Valid Physical Therapy license in applicable state(s) or proof of eligibility.
• Master’s degree in Physical Therapy or equivalent; or Bachelor’s plus 5 years of experience.
• Reliable transportation and valid driver’s license.
• Ability to adapt treatment plans, assess patient progress, and work independently.
