About the role
Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations using appropriate methods. Establishes a plan of care and implements individual treatment based on evaluation results and functional goals. Monitors and evaluates outcomes, adjusting the treatment plan as needed and providing status updates to physicians and other team members. Identifies and addresses factors affecting patient comfort and provides discharge planning, including written home programs, patient/family teaching, referral for community services, and transition planning.
Responsibilities
- Interprets physician referrals and completes evaluations
- Establishes and implements individual treatment plans
- Makes adjustments to treatment plans based on patient response
- Provides status updates to physicians and other team members
- Identifies and addresses factors affecting patient comfort
- Plans and completes patient discharge
- Completes daily statistics, billing, and scheduling
- Applies insurance guidelines regarding authorization and number of visits
- Maintains site/department established productivity standards
- Communicates and collaborates with appropriate stakeholders
- Completes required documentation following department policy and accreditation standards
- Records patient care and progress
- Participates in performance improvement activities
- Manages assigned caseload
- Orients new staff to clinical and department policies
- Participates in the evaluation process of co-workers
- May supervise physical therapy assistants/rehab aides/techs/physical therapy students
Requirements
- Licensed Physical Therapist in the State where employed
- Basic Life Support (BLS) for Healthcare Providers certification from the American Heart Association
- Doctorate degree in Physical Therapy from an accredited school or equivalent education
- Maintains continuing education requirements as mandated by applicable state professional standards
Qualifications
- Intermediate computer skills
- Excellent communication, interpersonal, and organizational skills
- Demonstrated organization, decision-making, and problem-solving skills
- Successfully completes all competency assessments/requirements pertaining to provision of patient care
- Knowledge of principles of growth and development over the life span
- Functional use of senses to allow for effective communication
- Ability to lift up to 50 lbs., lift up to 10 lbs. from waist to overhead, lift and carry up to 40 lbs. at waist height a reasonable distance, push/pull with 30 lbs. of force, perform a sliding transfer of 150 lbs. with a second person present
- Operates all equipment necessary to perform the job
