Educational Requirements

  • Graduated from an APTA accredited school of Physical Therapy, or equivalent.
  • Current Physical Therapy license within the State of practice.

Experience and Certifications

  • BLS certification required.
  • Actively enrolled in an accredited corresponding certification or residency program.

Skills and Knowledge

Must demonstrate clinical competency, experience, training, and education relevant to the job. Must be committed to professional development and able to become proficient with computer software applications commonly used in an automated patient care environment.

Responsibilities

  • Evaluates patient's functional abilities and limitations.
  • Determines if intervention is needed.
  • Establishes a plan of care and treatment goals.
  • Collaborates with patient and family/caregiver when setting goals.
  • Initiates discharge planning.
  • Recommends additions to or modifications of referring orders.
  • Implements individualized treatment plan with consideration to the patient's physical, social, spiritual, cultural, educational and age specific needs.
  • Provides skilled treatment in a safe manner.
  • Affirms ongoing effectiveness of treatment and modifies plan of care as indicated.
  • Involves patient and family/caregiver in the treatment and decision-making process.
  • Provides ongoing education to physician, nurse, patient, family/caregiver, and interdisciplinary team.
  • Works with specialty certified Physical Therapist mentor to ensure growth and advancement of advanced clinical decision making by physical therapy resident or certification program enrollee in preparation for passing the certification examination.
  • Documents according to professional practice guidelines set by regulatory agencies.
  • Communicates pertinent patient information to ensure the best possible follow through of care.
  • Communicates evaluation/treatment results and patient progress to physicians, nurses, patient, family/caregiver, and interdisciplinary team.
  • Defends treatment rationale with standard practices and/or current literature.
  • Exhibits adaptability in assignments as it relates to required 1:1 mentorship hours, caseload, scheduling, and staffing shifts.
  • Takes initiative to assist in other patient care areas and accepts responsibility for all patient care needs.
  • Demonstrates appropriate supervision and delegation of personnel to achieve safe delivery of quality cost effective patient care.
  • Participates in department operational activities.
  • Attends department required meetings and/or follows up to obtain pertinent information.
  • Participates in departmental/facility process improvement.
  • Mets productivity standards set by the department.
  • Participates with the development and implementation of evidence-based physical therapy guidelines/protocols.
  • Interacts with all levels of staff in a variety of departments, physicians, patients, families, and external contacts such as employees of other health care institutions, community provides and agencies concerning the health care of the patient.
  • Also interacts with the physician offices to report, request or clarify information.
  • Internal customers may include Patients, Nurses, Licensed Independent Practitioners, Nursing Directors, Department Directors, Administrators, and other health care team members for the purpose of integrating services, improving patient care, and ensuring effective communication systems.
  • External customers may include local, state, and national physical therapy organizations, Joint Commission, State Board of Physical Therapy, colleagues and peers, educational institutions, and physical therapy representatives.
