Responsibilities

• Provide skilled physical therapy services in the home setting
• Complete patient evaluations and develop individualized treatment plans
• Deliver therapeutic exercises, gait training, balance training, and mobility support
• Educate patients and families on home exercise programs and safety
• Complete accurate and timely documentation in compliance with agency standards
• Communicate progress and updates with the clinical team as needed

Qualifications

• Active New Jersey Physical Therapist license
• Strong communication and interpersonal skills
• Reliable transportation
• Able to work independently and professionally
