Physical Therapist (PT) Home Health - Full Time / Part Time (Per Diem)

About the role

We are seeking an experienced, self-driven Full-Time or Part-Time (Per Diem) Physical Therapist (PT) for Health Care to join our dedicated team and provide exceptional care throughout the region.

Responsibilities

• Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.

Requirements

• Current Physical Therapist (PT) License to practice in California - Required
• Experience in Home Health - 1 Year Required
• Current CA Driver’s License. Able to travel within a 15 - 25 mile radius on a regular basis.
