Physical Therapist (PT) - Full-time

Role: We are seeking a Physical Therapist (PT) to provide exceptional home-based patient care in Greeley, CO and surrounding areas. The position offers full-time and part-time opportunities, with visit rates ranging from $75-$150 plus mileage and travel time reimbursement. Responsibilities Conduct initial and ongoing comprehensive assessmentsProvide therapeutic interventions and treatmentsCommunicate effectively with interdisciplinary team members to coordinate care and discharge planningParticipate in Quality Assessment and Performance Improvement (QAPI) programsReport on patient conditions to the Rehab Director and staff Qualifications And Skills Current PT license in ColoradoBasic Life Support (BLS) CPR certification or willingness to obtainAt least one year of PT experienceHome health experience preferred but not requiredAbility to pass background checks and drug testing Ideal candidates should be organized, adaptable, self-motivated, and dependable, with strong clinical and communication skills, and a commitment to high-quality, compassionate patient care. Experience with OASIS documentation is preferred, with training provided if needed.
