Physical Therapist (PT) for Home Health

About the Role

FeldCare Connects is seeking a self-motivated Independent Contractor Physical Therapist to deliver premier excellence of care to adults of all ages in the San Jose area. The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs to promote the ability to move, reduce pain, restore function, and prevent disability. Based on this assessment, the Physical Therapist determines a treatment plan, executes interventions to improve patient well-being, and monitors progress effectively.

Responsibilities

• Assess and evaluate patient care needs related to mobility, pain reduction, function restoration, and disability prevention.
• Develop and implement individualized treatment plans based on patient assessments.
• Provide a variety of physical therapy treatments tailored to patient needs.
• Monitor and document patient progress with accuracy and discretion.
• Modify treatment plans as needed and communicate adjustments to relevant parties.
• Prepare and maintain detailed records of patient care and progress.
• Communicate effectively with patients, caregivers, and other healthcare professionals.
• Apply clinical reasoning to ensure safe and effective treatment planning and implementation.

Requirements

• Remarkable knowledge of physical therapy principles, theories, and methods in a rehabilitation environment.
• Exceptional ability to understand therapy in the context of a patient’s age, needs, and environment.
• Proven ability to communicate effectively, both written and orally.
• Sound clinical reasoning for treatment planning, implementation, and progress monitoring.
• Ability to recognize when patient protocols or treatment plans require modification.

Qualifications

• Physical Therapist license and registration by the state.
• Completion of a Physical Therapist curriculum approved by The American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, or The Council on Medical Education of the American Medical Association.
• Enthusiasm and ability to work with patients of all ages.
• Bilingual skills are a plus.

This is an in-home position requiring travel to patients' homes. While scheduling and documentation may be completed from your own home, the role is not remote.
