About the role

The Physical Therapist in Home Health delivers premier excellence of care to adults of all ages, focusing on assessment, evaluation, treatment plan development, intervention execution, and patient progress monitoring.

Responsibilities

  • Assess and evaluate patient care needs to promote movement, reduce pain, restore function, and prevent disability.
  • Determine a treatment plan based on assessment and evaluation.
  • Execute interventions aimed at improving and enhancing patient well-being.
  • Maintain accurate and discreet records.
  • Communicate effectively both orally and in writing.
  • Understand the role of therapy within the context of the patient's age, needs, and environment.
  • Prepare and maintain records with accuracy and discretion.
  • Recognize and modify patient protocols or treatment plans as needed to meet appropriate patient needs.
  • Report modifications to the therapist.

Requirements

  • Remarkable knowledge of physical therapy principles, theories, and methods in a rehabilitation environment.
  • Great skills in providing a variety of treatments.
  • Profound ability to communicate effectively both written and oral.
  • Exceptional ability to understand the role of therapy in the context of the patient's age, needs, and environment.
  • Exceptional ability to prepare and maintain records with accuracy and discretion.
  • Proven sound clinical reasoning in treating and planning, implementing, and monitoring patient progress.
  • Ability to recognize when patient protocols or treatment plans need modification to meet appropriate patient needs and report to the therapist.

Qualifications

  • Ability and enthusiasm in working with all ages.
  • Physical Therapist license and registration by the state.
  • Completion of a Physical Therapist curriculum approved by The American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, or The Council on Medical Education of the American Medical Association.
  • Bilingual a plus!

Pay

Compensation is above average and negotiable.

Schedule

You may complete your scheduling and documentation from your own home if you wish, though you will have to go to patients' homes for in-person visits.
