About the Role
The Physical Therapist provides direct rehabilitation care within a skilled nursing facility, focusing on long-term and short-term patient needs.
Responsibilities
- Develop, assess, and modify individualized treatment plans based on physician referrals
- Oversee and evaluate therapy staff and students
- Document patient progress accurately and timely
- Use therapy software effectively
- Collaborate with interdisciplinary teams to ensure quality care
- Maintain professional relationships, confidentiality, and compliance with laws and policies
- Participate in community health initiatives and support patient safety and rights
- Deliver quality physical therapy services and ensure compliance with standards
- Support patient recovery and functional improvement in a supportive environment
Requirements
- Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT)
- Current licensed or registered in the applicable state
- CPR certification (obtained upon hire or during orientation)
- Strong communication, body mechanics, and decision-making skills
- Proficient in Microsoft Office and email communication
- Able to work independently and as part of a team
