About the Role

The Physical Therapist provides direct rehabilitation care within a skilled nursing facility, focusing on long-term and short-term patient needs.

Responsibilities

  • Develop, assess, and modify individualized treatment plans based on physician referrals
  • Oversee and evaluate therapy staff and students
  • Document patient progress accurately and timely
  • Use therapy software effectively
  • Collaborate with interdisciplinary teams to ensure quality care
  • Maintain professional relationships, confidentiality, and compliance with laws and policies
  • Participate in community health initiatives and support patient safety and rights
  • Deliver quality physical therapy services and ensure compliance with standards
  • Support patient recovery and functional improvement in a supportive environment

Requirements

  • Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT)
  • Current licensed or registered in the applicable state
  • CPR certification (obtained upon hire or during orientation)
  • Strong communication, body mechanics, and decision-making skills
  • Proficient in Microsoft Office and email communication
  • Able to work independently and as part of a team
  • Commitment to professionalism, confidentiality, and patient-centered care
