Physical Therapist ( PT )

Full-time position, 40 hours per week on a day shift (9:30am - 6:00pm, Monday - Friday). Weekend coverage required one weekend every four weeks; holiday coverage required two holidays per calendar year.

About the role

This position is based in the House Rehab Services department in Muskegon, MI and provides physical therapy services on an as-needed basis at Shelby Hospital in Shelby, MI.

Responsibilities

• Provide treatment and services within the scope of licensure, certification, or registration and as required by law and regulation.
• Evaluate and clinically diagnose patient conditions.
• Establish treatment goals utilizing input from the patient and their family as appropriate.
• Maintain documentation in accordance with established internal/external timeliness standards, insurance standards, and reimbursement guidelines.
• Participate as part of an interdisciplinary and patient-centered care team, including verbal and written exchange of information with other healthcare team members to integrate a physical therapy treatment plan.

Requirements

• Graduate of a Doctorate’s or previously equivalent professional Physical Therapy training program accredited by the American Physical Therapy Association.
• Bachelor’s degree in Physical Therapy.
• FSBPT Certification.
• State of Michigan Physical Therapist License or eligible to obtain licensure prior to hire date.
• American Heart Association BLS or equivalent certification.
