Summit Home Health Care is hiring dedicated Physical Therapists to deliver home-based therapy to individuals supported through New Jersey DDD. We offer a therapist-driven model with leadership support, efficient documentation expectations, and a focus on mobility and independence in real-world settings.

Responsibilities

• Provide mobility and independence-focused treatment planning
• Deliver creative, individualized care in home-based settings
• Build strong therapeutic relationships with clients

Requirements

• Active New Jersey PT license
• Active CPR Certification
• Experience with neuro/developmental populations preferred
• Reliable transportation
• Ability to work independently
