About the Role
Summit Home Health Care is hiring dedicated Physical Therapists to deliver home-based therapy to individuals supported through New Jersey DDD. We’re looking for PTs who want flexibility, autonomy, and measurable impact in a therapist-driven model with leadership support.
This role is ideal for PTs seeking flexible supplemental work or transitioning from schools, outpatient, or facility-based settings.
Responsibilities
- Mobility and independence-focused treatment planning
- Creative, individualized care in real-world home settings
- Strong therapeutic relationship-building
- Efficient documentation
Requirements
- Active New Jersey PT license
- Active CPR Certification
- Experience with neuro/developmental populations preferred
- Reliable transportation
- Ability to work independently
