About the role

The Physical Therapist at Willow Tree Care Center will develop and implement a personalized physical therapy plan to help patients achieve their functional goals. They will work closely with other healthcare professionals and support staff to ensure optimal patient outcomes.

Responsibilities

• Evaluates functional needs and outcomes for patients
• Consults with other specialists to adjust care plans
• Designs and implements a plan of care based on thorough assessments
• Directs support staff to achieve patients' functional goals
• Communicates effectively with other professionals and support staff
• Updates documentation and maintains prescriptions and signatures
• Enhances professional growth and development of self and others
• Provides direction and leadership to staff
• Acts as a resource to promote physical therapy in the community

Qualifications

• Graduate of an APTA-approved physical therapy program
• At least one year of experience as a physical therapy aide
• Current license from the state Board of Physical Therapy Examiners
• Knowledge and skills to care for the physical and developmental needs of the population
• Ability to remain calm in emergency or crisis situations
