About the Role

As a Physical Therapist (PT) at Spring Creek Healthcare Center, you will be responsible for designing and implementing individualized physical therapy plans for our patients, with a focus on promoting independence, mobility, and overall well-being. Our patients are diverse and dynamic, and we need a Physical Therapist (PT) who is just as adaptable and creative in their approach. You will work closely with our interdisciplinary team to ensure comprehensive care and achieve the best possible outcomes for our patients.

Responsibilities

• Conduct thorough initial evaluations and ongoing assessments to identify patient needs and goals
• Develop and implement individualized physical therapy plans to achieve optimal patient outcomes
• Provide skilled physical therapy interventions, including exercises, modalities, and other treatments
• Collaborate with the interdisciplinary team to ensure comprehensive care and achieve patient goals
• Effectively communicate with patients, families, and healthcare professionals to ensure seamless care transitions
• Maintain accurate and timely documentation of patient progress and treatment plans
• Stay up-to-date with the latest research and best practices in physical therapy to continuously improve patient outcomes and quality of care
