About the role
We are seeking a skilled Physical Therapist to provide individualized care in a skilled nursing community.
Responsibilities
- Assessing, planning, and implementing personalized physical therapy programs to optimize patient progress.
- Collaborating with healthcare professionals and support staff to achieve positive outcomes.
- Maintaining accurate documentation of evaluations, treatment plans, and progress.
- Providing education and support to patients and their families.
- Ensuring compliance with safety, infection control, and resident rights policies.
- Leading and mentoring support staff, promoting professional growth.
Qualifications
- Graduate from an APTA-approved physical therapy program.
- Current state license.
- At least one year of experience as a physical therapy aide.
- Ability to communicate effectively and work independently.
- Knowledge of safety, infection control, and documentation standards.
