Physical Therapist (PT)

About the role

The Physical Therapist provides direct rehabilitation care in a skilled nursing facility, focusing on long-term support and short-term rehabilitation for diverse patient populations with orthopedic, neurological, and cardiopulmonary needs.

Responsibilities

  • Develop and implement personalized treatment plans aligned with physician referrals and regulatory standards
  • Assess patient progress, adjust goals, and oversee care provided by PTAs and support staff
  • Document patient care accurately and utilize therapy software effectively
  • Collaborate with interdisciplinary teams to promote patient recovery and well-being
  • Participate in community health initiatives and maintain professional relationships
  • Ensure safety, confidentiality, and compliance with policies, laws, and ethical standards

Qualifications & Skills

  • Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT)
  • Licensure in applicable state
  • CPR certification; experience preferred but new graduates are welcome
  • Proficiency in Microsoft Office and good communication skills
  • Ability to make independent decisions, demonstrate good body mechanics, and work as part of a team
  • Strong understanding of post-acute care practices, laws, and regulations

Key Duties

  • Follow treatment plans, evaluate and modify patient goals, supervise therapy support staff, maintain documentation, and provide excellent patient care in a supportive environment
