About the Role

We are excited to offer our new in-house model of Rehab at Hamilton Arms Center. Currently, we are adding a PRN Physical Therapist (PT) to our valuable team of therapists. At Hamilton Arms Center, we have a dedicated rehab team committed to providing compassionate, family-oriented short- and long-term rehabilitation as well as skilled nursing care. Responsibilities

• Screen, examine, and evaluate residents, including history, systems review, and application of appropriate tests and measures; interpret findings to establish a diagnosis, identify impairments, determine the predicted level of improvement and time required, identify precautions, and design a plan of care.
• Develop appropriate treatment goals and methods in collaboration with the resident and caregivers; implement the treatment plan and complete all related documentation.
• Supervise Assistants, temporary licensees, students, and support staff in accordance with state licensure requirements and professional standards.
• Attend and contribute to resident care, staff conferences, and other related meetings.
• Consult with and make referrals to other disciplines and agencies to address the comprehensive needs of the resident.
• Provide exceptional customer service, ensuring residents and families receive the highest quality of service in a caring and compassionate environment.
• Responsible for Clinical Programming.
• Maintain efficiency standards.

Requirements

• Graduate of an accredited Physical Therapy program.
• Must have a valid professional license by discipline for the state.
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• At least one year of experience in Long Term Care (LTC) preferred.
• Passionate about post-acute rehabilitation and the older adult population.
• Ability to relate and interact positively, effectively, and appropriately with residents, family, professional, and facility staff.
