About the Role

Collaborate with a multidisciplinary team to develop individualized treatment plans, provide therapy to address various diseases and impairments, and support patients' rehabilitation goals in a supportive environment with progressive modalities.

Responsibilities

• Develop and implement individualized treatment plans in collaboration with a multidisciplinary team
• Provide therapy to address a range of diseases and impairments
• Support patients in achieving their rehabilitation goals using progressive modalities

Requirements

• Graduate of an accredited therapy program
• Active state license for skilled nursing facilities
• Prior rehabilitation experience in a skilled nursing facility (SNF) preferred

• Ability to work effectively in a team environment
• Adapt treatment approaches to meet patient needs
• Deliver compassionate and patient-centered care
• Knowledge of progressive treatment modalities is advantageous
