Physical Therapist (PT)

We are seeking a dedicated full-time Physical Therapist to oversee resident assessments, treatment planning, and implementation in a skilled nursing and rehabilitation setting. Key Responsibilities Include Providing Physical Therapy services based on physician orders and initiating referrals as needed.Conducting resident screenings and evaluations for treatment needs.Developing and implementing individualized treatment programs.Ensuring compliance with confidentiality standards and departmental procedures.Documenting services accurately and communicating effectively with the care team.Performing specialized evaluations when indicated and following management directives. Qualifications Bachelor of Science in Physical Therapy from an accredited program.Successful completion of the National Certification Examination for Registered Physical Therapists.Current state licensure or eligibility.0-3 years of experience in long-term care preferred. Skills & Attributes Strong assessment and treatment planning skills.Knowledge of federal, state, and local regulations.Excellent communication and documentation abilities.Commitment to patient care and teamwork. Additional requirements: Background screening through the AHCA Care Provider Background Screening Clearinghouse is required.
