About the Role
The Physical Therapist provides direct rehabilitation services in a skilled nursing facility environment.
Responsibilities
- Develop and implement personalized treatment plans
- Monitor patient progress and oversee therapy staff
- Document care accurately and collaborate with an interdisciplinary team to enhance patient outcomes
- Maintain patient safety, adhere to policies, and ensure confidentiality
- Assist with patient evacuation and participate in community health initiatives
- Follow treatment plans, establish and modify goals, and evaluate care provided by therapy staff
- Chart promptly and utilize therapy software
- Demonstrate excellent customer service and maintain dependable attendance
- Communicate effectively within the team
- Perform physical tasks including sitting, standing, lifting, and moving intermittently during work
Qualifications & Skills
- Degree from an accredited physical therapy program
- Active physical therapy license
- Preferably one year of post-acute care experience
- Proficient in Microsoft Office
- Excellent communication skills and good body mechanics
- Ability to make independent decisions and work well in a team
- Demonstrate professionalism and integrity
- CPR certification required
